چکیده
Background: Dose calculation errors are one of the most common types medication impacting children and they can result in significant harm. Technology-based solutions, such as computerized provider order entry, effectively reduce dose issues; however, these technologies not always optimized, resulting potential benefits being fully realized. Methods: We analyzed pediatric dose-related patient safety event reports submitted to Pennsylvania Patient Safety Reporting System using a task-analytic approach that focused on information used calculation, during ordering, preparation or administration. From reports, we identified whether was impacted by error, type involved, technology optimization could have mitigated issue. Results: Of 356 reviewed, 326 (91.6%) met criteria for The meeting had following issue types: wrong (49 326, 15.0%), incorrect ordering (97 29.8%), calculated properly preparation/administration (180 55.2%). Most issues (219 67.2%). Analysis age group drug class also revealed interesting patterns. Technology optimizations potentially addressed 81.6% identified. Conclusion: While many healthcare facilities adopted health other devices support process, optimized address issues. Human factors–informed recommendations, checklist, test cases optimizing provided context findings.

متن کاملProgram Optimizations and Transformations in Calculation Form
The world of program optimization and transformation takes on a new fascination when viewed through the lens of program calculation. Unlike the traditional fold/unfold approach to program transformation on arbitrary programs, the calculational approach imposes restrictions on program structures, resulting in some suitable calculational forms such as homomorphisms and mutumorphisms that enjoy a ...
